Senior Community Mental Health Practitioner

This is an exciting opportunity to join Derbyshire Healthcare NHS Foundation Trust as a Band 6 Community Practitioner. The High Peak Older Adult Community Mental Health Team (CMHT) and Living Well Team are seeking an enthusiastic and experienced professional to join their friendly and supportive team based in Swadlincote.
Main Responsibilities- Support the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality.
- Work collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
- Undertake comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs.
- Support and motivate individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources.
- Relevant qualification for the post (Diploma in Nursing/Degree in Mental Health Nursing) or Degree in Occupational Therapy.
- Experience in working with Older people with functional mental health diagnoses.
- Ability to meet the travel requirements of the post.
- Good time management and ability to work independently, as well as part of a team.
