Patient Care Advocate

We are seeking an experienced Care Coordinator to join our team in Forth Valley. As a Care Coordinator, you will be responsible for coordinating care services to meet the specific needs of our clients. This includes matching care workers to clients based on their level and type of care required, as well as ensuring that care is delivered efficiently and effectively.
Responsibilities:
- Coordinate care services to meet client requirements
- Develop and maintain strong relationships with care workers and clients
- Conduct regular care assessments and reviews to ensure quality standards are met
- Maintain accurate records and reports
Requirements:
- NVQ/SVQ 2 or 3 qualification (not essential)
- Registered with the SSSC
- 2 years experience working as a care coordinator for a Domiciliary Care Agency (ideally)
- Knowledge of CQC/CI/CIW Compliance is essential
Salary and Benefits:
The salary for this role is £27,000 per annum, which is competitive for the industry and reflects the importance of this role in delivering high-quality care services.
