About the Role
This exciting opportunity is for a Band 6 Mental Health Liaison Nurse to join our older adults mental health liaison team at the Queen Elizabeth Hospital. You will provide vital support to patients in the emergency department and on inpatient wards, working closely with multidisciplinary teams to ensure seamless care transitions.
As a key member of our team, you will be responsible for conducting thorough biopsychosocial assessments, formulating effective care plans, and collaborating with various stakeholders to ensure patient safety and well-being.
Key Responsibilities
* Conduct timely and effective biopsychosocial assessments of individuals referred to the team from the emergency department and inpatient wards
* Formulate and implement appropriate care plans, considering all necessary sources of information
* Work collaboratively with multidisciplinary teams to carry out risk assessments and management, ensuring patient safety and well-being
* Liaise and consult effectively with acute trust personnel, Oxleas mental health teams, other mental health providers, community health services, and emergency services
Requirements
To succeed in this role, you will need:
- 2 years post-registration experience
- Good written communication skills
- Experience in risk management
Salary: £38,890 - £44,884 per annum (dependent on experience). This competitive salary reflects the importance of your role in delivering exceptional patient care.
