Job Description

We are seeking a highly skilled Senior Transitional Care Specialist to join our team at York and Scarborough Teaching Hospitals NHS Foundation Trust.

About the Role

This is an exciting opportunity for an experienced healthcare professional to develop and implement Transitional Care pathways, minimizing separation of mothers and babies. The successful candidate will be responsible for expanding our current Neonatal Outreach offering and ensuring that care pathways within Neonatal Transitional Care (NTC) and Neonatal Outreach are implemented and evaluated effectively.

Key Responsibilities
  • Develop and implement robust Transitional Care pathways to prevent avoidable admissions to the neonatal unit.
  • Expand on our current Neonatal Outreach offering cross-site.
  • Evaluate the effectiveness of care pathways within NTC and Neonatal Outreach.
Requirements
  • Demonstrated significant Professional Clinical knowledge and experience acquired at Band 6 or above.
  • Knowledge and understanding of BAPM standards of care, Maternity Incentive Scheme (MIS), and requirements for implementation of NTC.
  • Registered Neonatal Nurse and/or Midwife with educated degree level: post graduate certificate or diploma.
  • Evidence of continuing professional development.

The salary for this role is estimated to be around £43,077 - £53,619 per annum, depending on experience.
