Role Overview

We are seeking a skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live, working closely with our outreach services in Kingston to provide high-quality healthcare interventions to individuals experiencing homelessness.

Key Responsibilities

  • Deliver clinical services to individuals in need, tailoring care to their unique needs and circumstances.
  • Work collaboratively with our outreach team to identify and address the health needs of individuals experiencing homelessness.
  • Develop and implement innovative clinical interventions to improve health outcomes and reduce health inequalities.
  • Provide education and support to individuals on healthy lifestyle choices and disease prevention.

Requirements

  • Registered Nurse with a passion for delivering healthcare services to vulnerable populations.
  • Experience working with individuals experiencing homelessness or in a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholder groups.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality services.
