About the Role

We are seeking a highly skilled and compassionate Specialist Health Lead to join our team at Single Homeless Project. As a Specialist Health Lead, you will play a vital role in developing and implementing our health approach, ensuring that our clients receive the best possible care and support.

Key Responsibilities
  • Develop and embed SHP's health approach into our services, working closely with our Health team to ensure seamless delivery.
  • Support clients with underlying health needs to access and maintain healthcare, completing initial health screening tools and creating health support profiles.
  • Make referrals to and support clients in engaging with external services, such as dentistry and podiatry.
  • Conduct health mapping and use this information to influence partnerships with external services, helping to overcome barriers faced by those experiencing homelessness.
Requirements
  • A strong understanding of common health issues and barriers to health access faced by people experiencing homelessness.
  • Knowledge of local level health services and providers, including homeless health teams.
  • Skills in creating and monitoring support plans, with a focus on client safety and safeguarding.
  • Ability to advocate for and coordinate access to healthcare for clients with multiple disadvantages.
