Role Overview

We are seeking a highly skilled and dedicated Care Coordinator to join our team. As a Care Coordinator, you will be responsible for supporting and managing a care portfolio of clients, including regular touchpoints and client visits.

Key Responsibilities

  • Support and manage a care portfolio of clients, including regular touchpoints and client visits.
  • Work closely with clients and supporting carers to ensure a stable team of carers is always rostered in and well in advance.
  • Conduct ongoing carer supervisions and annual appraisals for all carers who work for your clients.
  • Identify and resolve issues arising to ensure continuity of care for your clients.
  • Deliver high-quality care and customer service excellence in all aspects of your work.
