**Job Summary**

We are seeking enthusiastic and reliable healthcare support workers to join our team in Wandsworth. As a healthcare support worker, you will play a vital role in providing high-quality care to our clients, working closely with our specialist nurses and other healthcare professionals to deliver tailored care plans.

**Key Responsibilities**

  • Provide high-quality care to clients in their own homes, including assistance with daily living activities, personal care, and medication management.
  • Work closely with our specialist nurses and other healthcare professionals to deliver tailored care plans and ensure seamless care transitions.
  • Contribute to the development and implementation of care plans, working collaboratively with clients, families, and healthcare teams.
  • Maintain accurate and up-to-date records of client care, including progress notes and medication administration records.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in healthcare support work.

**Requirements**

  • You must be over 18 years old and have the right to work in the UK.
