Job Summary

The Community React Team at Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust is seeking a highly skilled and experienced Band 6 Nurse to join our team. As a key member of our community nursing team, you will be responsible for providing high-quality, person-centered care to patients in their own homes.

Main Responsibilities
  • Provide intensive packages of care to meet the needs of patients in their own homes, acting as a clinical role model to ensure high standards are embedded in clinical practice.
  • Collaborate with the Hospital at Home Co-ordinator and Nursing Team leader to support clinical nursing developments, including IV therapies, new care pathways, and single point of access.
  • Coordinate the nursing team in the absence of the Team Leader, prioritizing workload and ensuring adequate cover.
  • Receive referrals on a daily basis, taking appropriate action to ensure patient needs are met in a safe and timely manner.
  • Communicate effectively with patients, families, and colleagues, using verbal and non-verbal communication skills to ensure understanding and empathy.

Requirements
  • Registered Nurse with extensive post-registration experience.

  • Basic keyboard skills and proficiency in using electronic patient records.
