About the Role:
Randstad is seeking a dedicated and compassionate Social Worker to join our team in Newcastle, UK. As a Social Worker on our Hospital Discharge Team, you will work closely with patients, families, and healthcare professionals to ensure that discharge plans are safe, effective, and meet the needs of each individual patient.
Key Responsibilities:
- Conduct Comprehensive Assessments: Conduct thorough assessments of patients' needs to determine the appropriate level of care and support required post-discharge.
- Develop and Implement Discharge Plans: Collaborate with healthcare professionals, patients, and their families to develop and implement discharge plans that meet the unique needs of each patient.
- Provide Support and Advocacy: Provide support, advice, and advocacy for patients and their families during the discharge process, ensuring a smooth transition to community care.
- Manage Records and Documentation: Maintain accurate and up-to-date records of assessments, care plans, and discharge arrangements, ensuring compliance with relevant legislation and policies.
Requirements:
- Qualifications: Qualified Social Worker registered with Social Work England.
- Experience: Proven experience in a social work role, ideally within a hospital discharge or similar setting.
- Skills: Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
- Knowledge: Knowledge of relevant legislation, policies, and procedures related to hospital discharge and adult social care.
