About the Role
EPUT is seeking a skilled and compassionate Community Health Specialist to join our team in Mid Essex. As a key member of our Dementia Intensive Support Service, you will provide high-quality care to individuals with dementia, working closely with healthcare professionals and families to develop personalized care plans.

Job Responsibilities
* Assess and evaluate patients' needs, developing comprehensive care plans that address their physical, emotional, and social well-being
* Collaborate with multidisciplinary teams to ensure seamless care delivery
* Provide education and support to families and caregivers on dementia management and care techniques

Requirements
* Registered Mental Health Nurse or General Nurse with postgraduate qualifications or experience in dementia care
* Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
* Strong assessment and planning skills, with experience in risk management and crisis intervention
* Current UK driving license and access to a vehicle for work purposes

Salary and Benefits
The estimated salary for this position is £40,000-£60,000 per annum, depending on experience. We offer a range of benefits, including seasonal ticket loans, NHS discounts, and flexible working arrangements.
