Job Summary:
We are seeking a skilled Care Needs Assessor to join our team at Devon County Council. As a Care Needs Assessor, you will be responsible for assessing the care needs of individuals and identifying suitable outcomes.
About the Role:
This is a full-time position, working 37 hours per week. You will be part of a multidisciplinary team, including a manager, social workers, occupational therapists, and other professionals.
Key Responsibilities:
- Assess the care needs of individuals and identify suitable outcomes.
- Build upon an individual's own strengths, accessing voluntary support, or commissioning a package of care.
- Work independently and as part of a team to achieve outcomes.
- Develop and maintain effective relationships with individuals, families, and other professionals.
- Contribute to the development of care plans and reviews.
Requirements:
- Robust knowledge of the Care Act 2014 and other relevant legislation.
- Confidence in assessment skills and ability to work independently.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Commitment to safeguarding and promoting the welfare of children and vulnerable adults.
