About the RoleWe are seeking a highly motivated and compassionate Support Practitioner to join our team in West Dunbartonshire. As a Support Practitioner, you will play a vital role in supporting individuals with their everyday lives, enabling them to achieve their own ambitions and goals.
**Key Responsibilities:**
- Support and monitor individuals with their healthcare needs, including administering medication and personal care
- Promote independence and individuality according to individuals' My Support Plans
