We are seeking a compassionate and skilled Vulnerable Population Advocate to join our team at Remedy Social Work.

About the Role

The successful candidate will be responsible for coordinating community care services with statutory and voluntary organizations, while identifying and assessing risks to vulnerable residents and families.

Key Responsibilities
  • Provide social work services to patients and their families in the community.
  • Perform Care Act assessments, Mental Capacity Assessments, and Safeguarding enquiries.
Benefits

A competitive salary of $70,000 - $90,000 per annum, depending on experience.
