We're privileged to work with some amazing people here at Homewood Care Home, providing a residential service for adults with learning disabilities. As a Care and Support Practitioner, you'll play a key role providing personalised support, enabling people to engage in a range of meaningful activities both out in the community and within our safe and comfortable environment. Key Responsibilities:
1. Provide person-centred support to individuals with learning disabilities, focusing on their strengths and promoting independence.
2. Support individuals with daily living tasks, including eating and drinking, taking medication, personal care, and communication.
3. Work collaboratively with colleagues and other professionals to deliver a high-quality service, ensuring a positive experience for individuals and their families.
4. Participate in and contribute to the development of care plans, ensuring individual needs are met and goals are achieved.
5. Demonstrate a commitment to promoting diversity, equality, and inclusion, ensuring our service is accessible and responsive to individual needs.
6. Maintain accurate and up-to-date records, adhering to data protection and confidentiality principles.
