Physiotherapist - Community and In-Patient Rehabilitation

We are seeking a highly motivated and dynamic Physiotherapist to join our Rapid Response and Intermediate Care Team in the community and in-patient rehabilitation unit. As a key member of our team, you will work closely with our Physiotherapists, Occupational Therapists, Nurses, and Support Workers to deliver high-quality patient care.
Main Responsibilities- Undertake 6-month rotations in Intermediate Care, Rapid Response, and In-patient Rehabilitation
- Conduct comprehensive initial assessments with patients to identify their needs and goals
- Develop and implement individualized treatment plans to promote patient recovery and independence
- Liaise with family members, relatives, and carers to ensure effective communication and collaboration
- Deliver evidence-based treatment interventions to achieve optimal patient outcomes
- Monitor patient progress and adjust treatment plans as needed
- Delegate tasks to Support Worker colleagues to ensure efficient and effective care delivery
- Participate in clinical meetings, team meetings, and in-service training to stay up-to-date with best practices and guidelines
