Physiotherapist - Community and In-Patient Rehabilitation

We are seeking a highly motivated and dynamic Physiotherapist to join our Rapid Response and Intermediate Care Team in the community and in-patient rehabilitation unit. As a key member of our team, you will work closely with our Physiotherapists, Occupational Therapists, Nurses, and Support Workers to deliver high-quality patient care.
Main Responsibilities- Undertake 6-month rotations in Intermediate Care, Rapid Response, and In-patient Rehabilitation to gain diverse experience and skills.
- Conduct comprehensive initial assessments with patients to identify their needs and goals.
- Develop and implement personalized treatment plans to achieve optimal patient outcomes.
- Collaborate with family members, relatives, and carers to ensure seamless care coordination.
- Deliver evidence-based treatment interventions and conduct follow-up review visits to monitor progress.
- Delegate tasks to Support Worker colleagues to optimize team efficiency.
- Participate in clinical meetings, team meetings, and in-service training to stay up-to-date with best practices.
