Job Title: NHS Specialist Health Nurse

We are seeking a highly skilled and compassionate NHS Specialist Health Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our Palliative Care Team, you will play a vital role in providing high-quality care to patients with complex needs.

Main Responsibilities:
  • Undertake specialist nursing assessments of clients with complex palliative care needs
  • Prioritize factors involved in patients' conditions through advanced clinical reasoning
  • Identify patients at risk and vulnerable, and enact the process outlined in policies for Care of Vulnerable Adults and Care of Vulnerable Children
  • Monitor, evaluate, and review clinical assessments and interventions within the multidisciplinary framework
  • Act as an expert nurse in palliative care for patients and their families
Requirements:
  • Registered Nurse with relevant post-basic qualifications
  • Achieved or working towards a first degree
  • Ability to work individually and as a team member
  • Basic computer literacy
  • Ability to negotiate with clinical professionals, patients, and their families
  • Ability to prioritize workload and manage time effectively
  • Experience of delivering evidence-based practice
  • Excellent verbal and written communication
Desirable:
  • Contemporary and relevant clinical knowledge and expertise
  • Ability to supervise and develop people
Experience:
  • Extensive years post-registration in an appropriate specialty
  • Experience of case or ward management
  • Experience of teaching clinical staff and patients and families
  • Demonstrate ability to initiate, evaluate, and sustain change management
Audit and Research:
  • Previous experience of audit, standard setting, and quality issues
  • Ability to incorporate research into practice and motivate others to do so
  • Experience of user involvement in service planning and delivery
Professional Requirements:
  • Take responsibility for and demonstrate evidence of own professional development
  • Maintenance of professional registration
  • Demonstrate commitment to development of palliative and EOLC
  • Understanding of equal opportunities
