We are seeking a highly skilled Clinical Pharmacist to join our team in delivering high-quality pharmaceutical care as part of a multi-disciplinary team within our GP practices. The successful candidate will play a key role in providing aspects of the pharmacotherapy service, working closely with healthcare professionals to optimize patient outcomes.

Key Responsibilities

• Deliver pharmaceutical care to patients, working collaboratively with healthcare teams to ensure safe and effective medication management.
• Contribute to the development and implementation of pharmacy services, including medicines management and clinical pharmacy initiatives.
• Provide education and training to healthcare professionals on pharmacotherapy and related topics.
• Participate in quality improvement initiatives to enhance patient care and service delivery.
• Collaborate with other healthcare professionals to develop and implement policies and procedures related to pharmacotherapy.

Requirements

• Master's degree in Pharmacy or equivalent qualification.
• Registration with the General Pharmaceutical Council as a practicing pharmacist.
• Relevant postgraduate qualification in clinical pharmacy or equivalent experience.
• Independent prescribing qualification or willingness to undertake training.
• Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
• Strong IT skills, with proficiency in relevant software applications.
