We are seeking a highly skilled Clinical Pharmacist to join our team in delivering high-quality pharmaceutical care as part of a multi-disciplinary team within our GP practices. This role will involve providing expert advice and guidance on pharmacotherapy services.

Key Responsibilities

  • Deliver pharmaceutical care to patients, working collaboratively with healthcare professionals to optimize medication management.
  • Contribute to the development and implementation of pharmacy services, ensuring they meet the needs of our patients and staff.
  • Provide education and training to healthcare professionals on pharmacotherapy and medicines management.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
  • Collaborate with other healthcare teams to ensure seamless care transitions and optimal patient outcomes.

Requirements

  • Master's degree in Pharmacy or equivalent qualification.
  • Registration with the General Pharmaceutical Council.
  • Relevant postgraduate qualification in clinical pharmacy or equivalent experience.
  • Independent prescribing qualification or willingness to undertake training.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Good IT skills and ability to use electronic patient records systems.
