Job Title: Care and Support Professional

Estimated Salary: £35,000 - £45,000 per annum.

About the Role:

Do you have a passion for delivering high-quality care and support? We are seeking a dedicated Care and Support Professional to join our team at Lincolnshire Partnership NHS Foundation Trust, where you will work with males of working age detained under the Mental Health Act, located at our 15-bedded Low Secure Inpatient Ward for assessment and treatment.

As a valued member of the multidisciplinary team, you will contribute to promoting patients' independence, quality of life, and well-being through collaborative efforts with service users, carers, healthcare professionals, and external services.

Key Responsibilities:
  • Design and deliver individualized care plans to support service users' recovery and rehabilitation.
  • Collaborate with the ward occupational therapist to coordinate group and individual interventions within the ward and community.
  • Engage with the multidisciplinary team to ensure seamless care delivery and coordination.

We prioritize ongoing professional development and offer regular opportunities for peer support, training, and supervision from the Rehab OT lead. Our team is committed to empowering our members to share their expertise and contribute directly to shaping mental health occupational therapy provision in the trust.
