HertsOne Ltd, a GP Federation serving South and West Hertfordshire, seeks a highly skilled Clinical Pharmacist to join our team. This role offers a unique opportunity to work within our Enhanced Primary Care Monitoring service, providing support and intervention to children and young people with ADHD between the ages of 6 and 18.

As a Clinical Pharmacist in this role, you will carry out holistic assessments of patients referred by HPFT CAMHS, report, analyse, and discuss information gained from these assessments with your GP supervisor, psychiatrist, or relevant services. You will also need to correspond with GP supervisors, multi-professional agencies, schools, families, children serviced, and voluntary/statutory services.

Key responsibilities include:

  • Carrying out holistic assessments of patients referred by HPFT CAMHS
  • Reporting, analysing, and discussing information gained from assessments
  • Corresponding with GP supervisors, multi-professional agencies, schools, families, children serviced, and voluntary/statutory services
  • Attending bi-weekly MDT meetings with senior team to discuss and review caseload regularly

The estimated salary for this position is £45,000-£60,000 per annum, depending on experience.
