About the Role

We are seeking an experienced Occupational Therapist to join our team in providing high-quality rehabilitation services to adults in the community. As a key member of our multidisciplinary team, you will be responsible for assessing and intervening with clients to prevent hospital admission or re-admission and facilitate early discharge home from hospital.

Key Responsibilities
  • Perform advanced specialized Occupational Therapy assessments and interventions as a key member of the multidisciplinary rehabilitation and admission avoidance pathways.
  • Hold a caseload of community clients within the Intermediate Care service using advanced clinical reasoning and goal setting to support the assessment and interventions provided using a client-centered approach.
  • Provide supervision and facilitate professional development of Specialist Occupational Therapists, Occupational Therapists, and Rehabilitation Support Workers and Occupational Therapy students as required.

Requirements
  • Degree in Occupational Therapy.
  • HCPC registration.
  • Experience of working with adults and older people with chronic conditions and complex co-morbidities.
  • Rehabilitation experience at minimum Band 6 in all core areas: neurology, orthopaedics, respiratory, and care of the elderly.
