This role involves supporting patients with chronic conditions, promoting healthy lifestyles, and working closely with senior nurses to ensure seamless care delivery. Key Responsibilities
  1. Deliver high-quality nursing care to patients with chronic conditions, including diabetes, asthma/COPD, and heart disease.
  2. Develop and implement health promotion programs to support patients in adopting healthier lifestyles.
  3. Collaborate with other healthcare professionals to ensure comprehensive care delivery.
  4. Care for patients presenting with simple and complex wounds.
  5. Provide guidance and support to women seeking information on family planning options.
Requirements

To be successful in this role, you will need:

  • RGN with current NMC registration and evidence of Continued Professional Registration.
  • Recognized qualifications in diabetes or chronic heart disease or respiratory medicine.
  • Diploma in General Practice Nursing.
  • Prior experience in Primary Care.
  • Certifications/Courses in Cytology, Travel Vaccinations, Immunizations.
Salary Information

The salary for this role is approximately £45,000 - £60,000 per annum, depending on experience.
