Key Responsibilities:
  • Provide personal care that respects individuals' dignity and autonomy.
  • Promote independence by using enabling approaches and supporting individuals to meet their outcomes.
  • Support individuals to connect with their communities and access local services and support.
  • Deliver personalized care through individual choice and control.
  • Communicate effectively with individuals, families, and professionals, documenting and reporting information as necessary.
