Job Summary

We are seeking a highly skilled and experienced pharmacist to lead our Post-Acute Medicine team at King's College Hospital. As a Clinical Pharmacy Lead, you will be responsible for delivering high-quality, patient-focused pharmaceutical care to patients across a range of specialties.

Key Responsibilities
  • Lead and manage a team of pharmacists to provide clinical pharmacy services to post-acute medical and stroke wards.
  • Develop and implement local guidelines and protocols to ensure seamless pharmaceutical care on discharge.
  • Work closely with multidisciplinary teams to promote high-quality, cost-effective patient care.
  • Provide interface and discharge liaison, and support wards & units within the Care Group.
  • Consistently lead on the provision of clinical pharmacy services to the many sub-specialities within the Care Group and ensure a high level of clinical pharmacy input.
  • Develop, implement and audit local guidelines.
  • Assist in the financial reporting to the Care Group and drug budget management.
  • Ensure seamless pharmaceutical care on discharge, liaising with other pharmacists and healthcare professionals (primary and secondary) taking over care of the patient.
Requirements
  • MPharm (Masters Degree)
  • Registered Pharmacist (1 year competency assessed and examined professional registration with the General Pharmaceutical Council)
  • Postgraduate Certificate in Pharmacy Practice or equivalent qualification/experience
  • Registered Independent Prescriber or willingness to complete
  • Member of the RPS faculty
  • Postgraduate Diploma in Pharmacy Practice or equivalent qualification / experience
