Job Title: Care Manager Home Care Specialist
Job Summary:
Voyage Care Ltd is seeking a highly skilled and experienced Care Manager to lead our residential service in Coventry. As a Care Manager, you will be responsible for managing the day-to-day operations of the service, developing and implementing person-centered support plans, and ensuring the highest quality of care for our residents.
Key Responsibilities:
- Manage the day-to-day running of the service, including supervising staff and ensuring compliance with regulatory requirements.
- Develop and implement person-centered support plans that meet the unique needs and goals of each resident.
- Conduct care needs assessments and create comprehensive care plans that promote independence and well-being.
- Recruit, induct, and train new staff members to ensure a high-quality care team.
- Complete supervisions, appraisals, and medication audits to ensure staff are competent and up-to-date with best practices.
- Attend multi-disciplinary meetings and collaborate with local authorities, healthcare professionals, and families to ensure seamless care coordination.
- Manage the service's budget and ensure financial sustainability.
- Ensure regulatory, legislative, internal, and contractual requirements are met, including relevant notifications and adherence to policies and procedures.
Requirements:
- Previous experience as a CQC registered manager or in a care management role.
- Willingness and ability to undertake CQC registration.
- Level 5 in Health & Social Care or equivalent qualification.
- Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds, and challenging behaviors.
