Geriatric Medical Practitioner - Clinical Care Coordinator

We are seeking a highly skilled Geriatric Medical Practitioner to join our team as a Clinical Care Coordinator at The James Cochrane Practice. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, person-centred care to residents in care homes across Kendal.

You will work closely with lead care home clinicians, general practitioners, and other healthcare professionals to ensure seamless delivery of holistic patient care. Key responsibilities include providing expert clinical care and management of frail and older adults in care homes, collaborating with a well-established Care Home multidisciplinary team, and promoting best practices in care of older adults.

We strive for a good work-life balance while achieving high-quality patient care and offer a fair allocation of work across all the GP team. We have a Quality Improvement culture and continually develop our systems to respond to the changing environment of primary care.
