We are seeking a skilled and compassionate Reablement Support Worker to join our expanding Discharge to Assess (D2A) Team at Candlelight Care.

Job Summary:

As a Reablement Support Worker, you will play a vital role in providing short-term care and reablement services to patients in their own homes, bridging the gap between hospital and home.

Key Responsibilities:

  • Deliver high-quality care and support to patients, promoting independence and well-being.
  • Work collaboratively with healthcare professionals to ensure seamless transitions and efficient patient flow.
  • Conduct assessments and develop personalized care plans to meet individual needs.
  • Provide education and support to patients and their families on self-care and management techniques.

Requirements:

We are looking for a motivated and empathetic individual with a passion for delivering exceptional care. Previous care experience is desirable but not essential. You will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

What We Offer:
