Clinical Pharmacist

The Clinical Pharmacist will work within the Primary Care Network to provide expertise in clinical medicines management, including face-to-face structured medication reviews, management of long-term conditions, and transfer of care. The post holder will also provide leadership on quality improvement and clinical audit around medications, contributing to the Quality and Outcomes Framework and local contract performance indicators.
Main Duties- Review patients with single or multiple medical problems where medicine optimisation is required, and make recommendations to Senior Pharmacists or GPs for medicine improvement.
- Manage caseloads of patients with common/minor/self-limiting ailments, working within a scope of practice and limits of competence.
- Provide patient-facing clinics for those with questions, queries, and concerns about their medicines in the practice.
- Implement changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.
- Monitor practice prescribing against the local health economy's RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors or subject to shared care.

Key Responsibilities- Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
- Work with care home staff to improve the safety of medicines ordering and administration.
- Attend and refer patients to multidisciplinary case conferences.
- Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
The ideal candidate will have a commitment to reducing health inequalities, excellent communication skills, and the ability to work flexibly and enthusiastically within a team. They will also have a minimum of two years' experience as a pharmacist, demonstrated within a practice portfolio, and experience of partnership/collaborative working and of building relationships across a variety of organisations.
