We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Rochdale Health Alliance. As a Clinical Pharmacist, you will play a key role in providing expert medication management services to our patients, working closely with our multidisciplinary team to optimize patient care.
Key Responsibilities- Conduct clinical medication reviews with patients to identify areas for improvement and provide recommendations for optimized medication regimens.
- Work with patients, caregivers, and healthcare professionals to develop personalized medication plans that meet their unique needs and goals.
- Provide education and support to patients and their families on medication management, including proper use, side effects, and potential interactions.
- Collaborate with our primary care network to develop and implement medication management strategies that improve patient outcomes and reduce healthcare costs.
- Stay up-to-date with the latest medication management guidelines, research, and best practices to ensure that our patients receive the highest quality care.
- Registration with the General Pharmaceutical Council (GPhC) as a pharmacist.
- Minimum of two years' experience as a pharmacist, preferably in a primary care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, caregivers, and healthcare professionals.
- Strong analytical and problem-solving skills, with the ability to interpret complex medication data and develop effective solutions.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
