Job Summary

We are seeking an experienced Health and Wellbeing Care Professional to join our Community Mental Health Service team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our team, you will play a vital role in providing high-quality care to our patients.

Your primary responsibilities will include assessing and managing clinical risk, delivering evidence-based interventions, and working collaboratively with multidisciplinary teams. Key Responsibilities
  • Delivery of evidence-based interventions in mental health
  • Assessment and management of clinical risk
  • Working within a multi-disciplinary team
  • Managing own case load
Requirements
  • A relevant professional qualification relating to Health or Social Care (e.g., nursing, OT, social work)
  • Recent evidence of continuing professional development
  • Able to meet the travel requirements of the post
What We Offer
  • Estimated salary: £30,000 - £45,000 per annum
  • 27 days leave plus bank holidays, increasing up to 33 days with long service
