About the Role

We are seeking a dedicated and proactive Community Links Worker to join our team at We Are With You. As a Community Links Worker, you will be uniquely placed to support individuals in areas of high deprivation, where health inequalities are prevalent.

Through one-to-one support appointments and small group work, you will help people overcome their health and wellbeing challenges. Your role will involve connecting individuals with community support services, addressing issues such as low-level mental health difficulties, housing problems, poverty, relationships, and bereavement.

As a frontline worker, you will deliver continuity of care through effective partnership working, intervention delivery, and key work responsibilities. This is an integral role for our organization, and we are looking for someone who is passionate about making a difference in the lives of others.

Key Responsibilities
  • Deliver one-to-one support appointments and small group work to individuals in need
  • Connect individuals with community support services to address health and wellbeing challenges
  • Work effectively with the multidisciplinary team in the GP Practice to deliver continuity of care
  • Deliver interventions and key work responsibilities to support individuals in achieving their goals
