Job Summary
We are seeking a highly motivated and experienced Social Worker to join our Integrated Adult Social Care team in Devon. As a key member of our team, you will be responsible for assessing and reviewing eligible needs under the Care Act, supporting MCA-compliant decision making, and ensuring the safeguarding of adults at risk of abuse.
About the Role
This is a full-time position requiring office-based working at least two days per week. You will be working with young people from 17 years old transitioning to Adult Social care through to adults aged 65 who present with a range of physical and/or learning disabilities, as well as enduring health conditions.
Key Responsibilities
- Assess and review eligible needs under the Care Act
- Support MCA-compliant decision making
- Ensure the safeguarding of adults at risk of abuse
- Collaborate and negotiate with service users, their families, providers, and other healthcare professionals
- Proactively encourage independent and safe living within the community
