We are seeking a dedicated and compassionate Support Worker to join our team in the West Kent locality. As a Support Worker, you will play a vital role in supporting individuals with a learning disability to access and participate in their local community.
Key Responsibilities:- Provide person-centred support to individuals with a learning disability, promoting their social inclusion and rights.
- Work collaboratively with service users, families, and other professionals to develop and implement individualised support plans.
- Assist individuals with daily living tasks, such as personal care, meal preparation, and household management.
- Support individuals to access community resources, such as education, employment, and leisure activities.
- Develop and maintain relationships with service users, families, and other professionals to ensure effective communication and collaboration.
- Diploma Level 2 in Health and Social Care or equivalent.
- Relevant experience of care and support of individuals with learning disabilities.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Enhanced Disclosure Application to the Disclosure and Barring Service.
