About the Role

We are seeking a highly motivated and empathetic Community Wellbeing Facilitator to join our Active Lives Service. In this role, you will work closely with adults with severe learning disabilities, autism, and other complex needs, providing support and guidance to enhance their overall wellbeing.

Our service is built on the principles of Community Led Support, prioritising person-centred approaches that empower individuals to take control of their lives. As a Community Wellbeing Facilitator, you will play a vital role in delivering sessions that promote emotional and physical wellbeing, social inclusion, and independence.

You will work in partnership with service users, families, and carers to develop personalized plans that address their unique needs and goals. This may involve collaborating with external agencies, advocacy groups, and healthcare professionals to ensure comprehensive support.

Key Responsibilities:

  • Deliver person-centred support to adults with severe learning disabilities, autism, and other complex needs.
  • Develop and maintain strong relationships with service users, families, and carers.
  • Collaborate with external agencies, advocacy groups, and healthcare professionals to provide holistic support.
  • Contribute to ongoing evaluation and improvement of services, ensuring high-quality outcomes for service users.

Salary Range: £31,986 - £32,931 (dependent on experience)
