About the Role

We are seeking an experienced Cardiac Specialist/Heart Failure Nurse to join our team at Chelsea and Westminster Hospital NHS Foundation Trust. As a key member of our cardiac care team, you will provide expert care to patients with heart failure, working closely with our multidisciplinary team to deliver high-quality patient outcomes.

Key Responsibilities
  • Manage your own caseload, including nurse-led clinics, virtual clinics, telephone consultations, and heart failure exercise programs.
  • Provide support and education to cardiac inpatients within the hospital.
  • Contribute to the development of cardiology services, both internally and externally, through leadership and collaboration.
  • Analyze service performance through regular audits, quality improvement activities, and strategic development.
  • Disseminate experiences through publications, conference presentations, and participation in local heart failure and nursing research, as well as national working parties.
