About the Role

We are seeking an experienced and enthusiastic Clinical Nurse Specialist to join our team at Homerton University Hospital. As a key member of our heart failure service, you will play a vital role in providing high-quality care to patients with chronic heart failure.

Key Responsibilities
  • Lead on specific areas to ensure patients have a seamless pathway through different aspects of the service
  • Support heart failure patients to optimize their treatment and manage their condition
  • Prevent unplanned admissions and work closely with the hospital team
  • Develop personal management plans and educate patients and carers for increased confidence in self-care
Requirements
  • RGN/RN qualification
  • Post-registration nursing course in cardiac or heart failure care
  • Recognized teaching certificate
  • Clinical assessment skills
  • Experience of caring for patients with chronic or acute heart failure
  • Venepuncture training and proficiency
  • Significant post-registration experience in cardiology
  • Understanding and demonstration of the Trust Values
Desirable Criteria
  • Independent non-medical prescriber
  • Advanced physical assessment course
  • Experience of working in primary care
