About the Role

Alliance for Better Care CIC is a GP Federation that unites 47 NHS GP practices across 12 Primary Care Networks in Sussex and Surrey. We support our Primary Care colleagues as well as their patients, to transform how healthcare is managed within the community.

Job Summary

We are seeking a skilled and compassionate healthcare professional to join our equity team. As a Healthcare Equity Specialist, you will play a vital role in delivering services to vulnerable cohorts and communities who face additional barriers to accessing healthcare.

Key Responsibilities

  • Deliver health assessments, immunizations, health checks, and health promotion services to vulnerable populations.
  • Work collaboratively with the team to provide person-centered care plans and support access to local health services.
  • Liaise with partners to make referrals, signpost, and support access to social prescribers, maternity, mental health, and primary care services.
  • Provide information, advice, and support to vulnerable cohorts on social, physical, and mental health wellbeing.
  • Participate in safeguarding processes and adhere to ABC's procedures and Surrey/Sussex Safeguarding Adults and Children's procedures.

Requirements

  • Registered General Nurse (Adult and Child), Registered Midwife (Adult and Child), or Registered Paramedic with a minimum 5 years' experience.
  • Community-experienced nurse/paramedic with experience of working in primary or urgent care.
  • Excellent communication and interpersonal skills, with the ability to work autonomously and prioritize workload.
  • Valid license to drive in the UK with access to a car for business use.
