About the Role

We are seeking a highly motivated and compassionate individual to join our team as a Support Worker Outreach. As a key member of our outreach team, you will play a vital role in supporting vulnerable adults who are experiencing homelessness.

Key Responsibilities

  • Provide outreach support to individuals who are sleeping rough or vulnerably housed, promoting safety and creating pathways into accommodation.
  • Work collaboratively with statutory and voluntary partners to provide a comprehensive support service.
  • Assess the needs of individuals and develop personalized support plans to help them achieve their goals.
  • Provide emotional support and guidance to individuals, helping them to build confidence and independence.
  • Record and report essential data to inform service development and improvement.

Requirements

  • Knowledge of homelessness and the key needs experienced by vulnerable homeless people.
  • Excellent communication and IT skills.
  • Ability to work as part of a team and build strong relationships with colleagues and partners.
  • Understanding of and commitment to the spiritual principles of The Salvation Army.
