Adult Care Specialist

We are seeking a qualified Adult Social Worker to join our team in Carlisle, UK. As an experienced professional, you will be responsible for conducting high-cost transport reviews for adults in need.
This role involves assessing and reviewing current transport arrangements, ensuring effective and sustainable solutions that balance cost efficiency with maintaining access to essential services. You will work closely with service users, families, and providers to develop and implement revised transport plans, promoting independence and cost-effectiveness.
Key responsibilities include:
- Conducting thorough assessments of transport provisions for adults with care needs
- Managing your own diary and coordinating with multi-agency teams
- Developing and implementing revised transport plans, promoting independence and cost-effectiveness
