We are seeking an experienced Children's Complex Care Nurse to join our team at Thornbury Community Services. As a Children's Complex Care Nurse, you will be responsible for delivering bespoke care and support to young people with complex needs, enabling them to stabilise at home and achieve their personal goals.

Key Responsibilities:

  • Deliver high-quality care and support to young people with complex needs
  • Work collaboratively with the multidisciplinary team to develop and implement individualised care plans
  • Provide education and training to families and carers on complex care needs
  • Participate in the development and implementation of policies and procedures related to complex care

Requirements:

  • Minimum of 6 months' experience in the last 2 years in a similar role
  • Strong knowledge of complex care needs and best practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
