Salaried General Practitioner

The Willow Group is seeking a highly skilled Salaried General Practitioner to join our dynamic team. As a key member of our primary care team, you will be responsible for providing high-quality patient care and contributing to the development of our population-based health and social care services.
Key Responsibilities- Provide autonomous decision-making in relation to presenting problems
- Consult with patients in the surgery, at home visits, and via telephone or online mechanisms
- Undertake triage calls, triage visits, and checking and signing of repeat prescriptions
- Screen patients for disease risk factors and early signs of illness
- Develop care plans for health in consultation with patients and in line with current practice disease management protocols
- Provide counseling and health education as necessary
- Admit or discharge patients to and from caseload and refer them to other care providers internally and externally as appropriate
- Comply with all relevant clinical governance and practice policies and protocols
- Assist in the establishment of appropriate systems to manage common chronic medical conditions
- Record clear and contemporaneous consultation notes to agreed standards
- Provide active input to the wider primary healthcare team in the process of health needs research, baseline data collection, and clinical audit
- Compile and issue computer-generated acute and repeat prescriptions, prescribing in accordance with the practice/Fareham & Gosport CCG prescribing formulary
- Meet professional learning and CPD requirements
