We are seeking a dynamic and creative clinician who wants to make a difference in the lives of families and provide exceptional care.

Main Responsibilities
  • Manage a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
  • Participate in setting quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, adhering to relevant Trust procedures and NMC Guidance.
  • Assess patient care needs, develop, implement, and evaluate programs of care.
  • Deliver a range of interventions, including pharmacological and psychologically informed interventions, in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans, promoting positive risk-taking to improve quality of life and maximize occupational performance/independence.
  • Maintain an effective reporting system, observing and reporting verbally and in writing on patient condition.
  • Utilize agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
