Overview
Cyngor Sir Ceredigion County Council is seeking a highly skilled and experienced Senior Children Services Practitioner to join their team. The successful candidate will be responsible for supporting vulnerable individuals and their families in hospital and community settings.

Key Responsibilities:
- Support vulnerable individuals and their families in hospital and community settings
- Apply a Strength-based needs assessment focusing on service user choice and control using the Signs of Safety framework
- Provide information, advice, and assistance about community health and social care services for adults

Salary: £40,000 - £45,000 per annum (dependent on experience)
