Job Summary

We are seeking a highly skilled and experienced Exercise Programme Coordinator to join our team at Great Western Hospitals NHS Foundation Trust.

The successful candidate will be responsible for delivering a structured evidence-based exercise programme to patients with cardiac conditions, working closely with specialist nurses and exercise therapists to achieve tailored, person-centred goals.

Main Responsibilities

Deliver a structured evidence-based exercise programme as screened and delegated by qualified therapists, in line with agreed local pathways and national targets.

Work in partnership with specialist nurses and exercise therapists to support patients in achieving their goals, reviewing and adjusting interventions as needed.

Assist with sub-maximal exercise assessments for a range of cardiac conditions and contribute to the effective reporting of feedback, incidents, and complaints.

Service Improvement

Work with colleagues to develop current and new services, dealing with requests from patients and clinical staff for health information leaflets, and participating in organisational activities to improve patient care.

Confidentiality

Ensure the confidentiality of sensitive information relating to patients, carers, colleagues, and other healthcare workers, adhering to practice policies and procedures.

Health and Safety/Risk Management

Comply with Great Western Hospitals NHS Trust Health and Safety policies, following safe working procedures and reporting incidents using the Trust's Risk Incident Reporting System.

Equality and Diversity

Support the equality, diversity, and rights of patients, carers, and colleagues, respecting their privacy, dignity, needs, and beliefs.

Personal/Professional Development

Participate in training programmes, maintaining a record of personal and professional development, and taking responsibility for own development, learning, and performance.

Maintain quality within the practice, alerting senior team members to issues of quality and risk, and contributing to the effectiveness of the team by reflecting on own and team activities.

Communicate effectively with team members, patients, and carers, recognising the importance of peoples' needs for alternative methods of communication.
