About the Role

Cornerstone Community Care is seeking a dedicated and compassionate Support Practitioner to join our team. As a Support Practitioner, you will play a vital role in providing support to adults with various disabilities and/or additional support needs in Perth.

Key Responsibilities

  • Provide personal care and support to individuals with their day-to-day needs, including personal care, shopping, finances, cooking, housework, and medication.
  • Support individuals to participate in and engage with their local community, including activity clubs, swimming, football, boccia, day trips, and eating out.
  • Work collaboratively with the team to develop and implement personalized support plans that meet the unique needs and goals of each individual.
  • Communicate effectively with individuals, families, and other stakeholders to ensure that their needs are met and their rights are respected.
  • Contribute to a positive and inclusive team culture that values diversity, equity, and inclusion.

Requirements

  • Confidence to work independently and as part of a team.
  • A caring and compassionate attitude, with a strong commitment to supporting individuals with disabilities and/or additional support needs.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and other stakeholders.
  • Ability to work flexibly, including split shifts and evening/weekend work.
  • Good administration skills, with the ability to maintain accurate records and reports.
