We are seeking an experienced CHC Nurse Assessor with expertise in Learning Disabilities to join our team at Real Staffing. This is an exciting opportunity to work with our NHS client in the East Midlands on a 3-month contract, offering hybrid working arrangements.
Key Responsibilities:- Conduct comprehensive assessments of individuals with complex needs, ensuring compliance with national standards and guidelines.
- Provide guidance on care packages that meet individual requirements, utilizing your knowledge of Continuing Healthcare frameworks.
- Collaborate with other healthcare professionals to complete joint funding reviews, ensuring timely review processes are followed.
- Registered Learning Disability Nurse with a minimum of two years' experience as an CHC Nurse Assessor.
- Excellent communication skills, both verbal and written, with the ability to demonstrate compassion towards patients.
- Knowledge of National Frameworks, such as DST/tools/LCP, is essential.
