Job Overview

The successful candidate will be responsible for providing face-to-face support to clients and their families, while creating personalized care plans tailored to each individual's needs. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.

Key Responsibilities:
  • Design and deliver tailored care plans specific to an individual client's needs
  • Effectively manage the multidisciplinary team through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of client's physical and physiological needs within your caseload
  • Deliver nursing interventions, including the management of medicine delivery
  • Build and maintain sophisticated relationships with your client's family
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
Requirements:
  • RSCN with an active NMC PIN
  • Supportive management style that can bring out the best in others
  • Up-to-date experience of tracheostomy care and ventilators
  • Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK Driving Licence essential
Salary and Benefits:
  • Up to £43,680 per annum DOE
  • Monday to Friday (9-5) hours
  • KPI Bonus 8% of annual salary
