Job Summary

As a Healthcare Support Worker at St Wilfrid's Hospice, you will be responsible for:

  • Observing patients and providing care in accordance with management plans under the supervision of a Team Leader
  • Working as part of a multi-professional team to deliver palliative nursing care interventions
  • Maintaining accurate records and communicating effectively with patients, families, and healthcare professionals

Key Responsibilities

• Provide high-quality care to patients in their homes, in accordance with management plans and under the supervision of a Team Leader

• Work collaboratively with other healthcare professionals to deliver comprehensive care and support

• Maintain accurate records and reports, and contribute to the development of care plans

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery

Person Specification

• Level 3 Diploma in Health and Social Care (or equivalent) or working towards

• Recent experience in a healthcare setting, preferably in general/community nursing

• Ability to demonstrate good communication skills, work autonomously, and maintain accurate records

• Ability to work in the community and have access to a car

• Valid or prepared to obtain business class insurance for vehicle
